What You Need to Know
• All patients will need to create a new username and password for the new portal.
• If you currently have proxy access to a loved one’s account, you will need to re-establish proxy access in the new portal.
• Children ages 14–17 can sign up for their own account by visiting AveraMyChart.org.

With the new portal, you can:

• Schedule primary care appointments.
• View health insurance and benefit information.
• Complete virtual visits.
• View medications and lab results.
