Community Memorial Home Health provides in home care and nursing services in the Redfield and surrounding area. The staff work closely with the client, his or her family, and the medical provider to design a plan of care to meet each client’s specific needs.
Depending on the plan of care designed for the client, Community Memorial Home Health will send staff specifically trained to meet those needs, all in the comfort of your own home.

Nursing Service
Managing health conditions at home is a priority to many of our clients. Some of the services we provide are:
- medication management
- health assessment
- monitoring vitals
- dressing changes
- ostomy/colostomy education
- nail care
- home infusion education
- support and infusion site management
- NG nutritional support
- post op care
- foley catheter insertion and care
- routine blood draws
- sleep studies
Therapy Services
Our Home Health Therapy Team consists of Physical Therapy, Occupational Therapy, and Speech Therapy.
Personal Care
Many of our clients require assistance with personal care. Whether this is assistance in dressing, bathing, completing a home exercise program, preparing meals, running errands, or medication reminders; we are here to help you meet your goals.
Homemaking
Maintaining your home can become overwhelming. Let us help you with your laundry and housekeeping chores.
Respite Care
Spouses and adult children are usually the first to step in and help their loved one with certain tasks. In home respite care can provide respite services to your loved one while you are away tending to your own personal needs and or appointments.
Foot Care Clinic
Community Memorial Home Health has a Foot Care Clinic the second and fourth Tuesday of each month by appointment.
