Patient Admissions

Welcome to Community Memorial Hospital Patient Admissions Department. Our goal is to make your registration process as pleasant and convenient as possible. Accuracy, efficiency, and patient confidentiality are the primary focus of this department while delivering the best in customer service. Patient Admission includes the information desk, registration area, switchboard and centralized scheduling.

Any test or procedure done in the hospital requires patients to provide demographic information and to sign the appropriate consent forms. Please arrive at the hospital prior to your scheduled appointment time; the first available admitting clerk will check you in, in the order you have arrived.

Please have the following items with you when you arrive:

  • All current health insurance information/health insurance cards
  • Photo ID/Drivers License or Passport for patient verification purposes
  • Optional: A copy of your Living Will and/or Durable Power of Attorney to have on file in your medical record

7:00 a.m. - 4:30 p.m. Monday - Friday
